About the job Social Worker
The Young Adults Team work with young people with disabilities aged 16-25. The work is primarily with young people with Learning Disabilities, but also some young people with a physical disability, neuro-diverse needs or complex health needs, to transition from Childrens Services into their adult life. The work is very varied, and requires working in a multi-agency manner with Worcestershire Children First (WCF), Adult Services, Special Educational Needs Department (SEND), health colleagues, schools and colleges to ensure young people get the whole range of options they deserve when moving into adulthood. The work involves using Liquidlogic in both the adult and childrens systems and working within children and adults legislation effectively.
